This puzzle is part of the Stone Pedestal, and essential if you want to obtain the Depraved Idol treasure. As a prerequisite, you need to defeat Del Lago. This is so that you can get a new boat to explore the lake.

How to solve the hexagon puzzle in Resident Evil 4 Remake 

You can find the Hexagon Pieces in different areas across the lake. As mentioned, you can access them after defeating Del Lago and acquiring a new boat. Note that you do not need any keys to get the Pieces.

Here are the locations of all the Hexagon Pieces:

• Hexagon Piece A: In the Treasure Chest across from the Merchant at the start of Chapter 3.
• Hexagon Piece B: In the Fish Farm near where you pick up the gas container. To get it, you must go into the water when coming out of the house, take a left under the pathway, and head straight back to find the chest containing the piece.
• Hexagon Piece C: In the Small Cave Shrine at the northeast end of the lake. 

After collecting all three pieces, you must return to the Stone Pedestal and put them into the Pedestal in order, from A to B to C. Your goal is to make the image visible. The solution is to rotate the bottom left group of three once, followed by rotating the top group of three once.
